Member - Termination Request Form

This form is for those with active memberships, or memberships on hold at the Peterborough Sport & Wellness Centre who would like to terminate their membership. This form will allow you to request a termination up to 3 months in advance, with an effective date of termination beginning in July, August, or September. If you would like to terminate your membership after September, please visit the Centre to fill out a paper copy of this form.

To complete this form, please have your Member ID ready, which can be found on your key tag or member ID card.

Personal information is being collected under authority of the Municipal Act 2001, c. 25, and will be used for the purposes of managing your account/memberships and any transactions that you make (e.g. registrations and bookings), communicating with you regarding these matters, and administering and managing City-run programs and facilities.
